Open surgery at the lowest part of the spine, over the sacrum, in which part of the bony covering is removed, the lining around the nerves is opened, and an abnormality lying inside that lining is taken out. What is removed is not a tumor: it may be a fluid-filled cyst, a pocket of scar or inflamed tissue tethering the nerves, or a tangle of abnormal blood vessels. The lining is closed again once the work is done.
