A surgeon cuts directly into the ureter — the tube draining urine from the kidney to the bladder — and passes a small endoscope through that opening to burn away or cut abnormal tissue inside it, taking a tissue sample if something suspicious is seen. Reaching the ureter this way is chosen when it cannot be treated from below, or when the surgeon is already operating in that area. Growths on the lining and scarred narrowings are the usual targets.
