Treats a hand starved of blood because a dialysis access in the arm is diverting flow away from it. The surgeon builds a short bypass around the access to feed the hand, and ties off the artery just below the access so blood is no longer pulled backwards. This keeps the dialysis access usable while the hand gets its circulation back.
