Open-chest replacement of the curved top section of the main artery leaving the heart, where the vessels to the head and arms branch off. The damaged arch is cut out and a fabric tube sewn in, with those branch vessels reattached to the new tube. The heart-lung machine takes over the circulation, and blood flow to the brain is protected by separate measures while the arch itself is open.
