Utah rates for HCPCS 33465

Removes the tricuspid valve, which sits between the upper and lower chambers on the right side of the heart, and puts a replacement valve in its place, with the patient supported on a heart-lung machine. It is done when the valve leaks badly or is too damaged to repair. The replacement may be a tissue valve or a mechanical one.
