Open-heart surgery that replaces the aortic valve and at the same time enlarges the ring of tissue the valve sits in, by cutting into the wall of the heart's pumping chamber to widen it. It is done when that ring is too small to accept a valve of adequate size, which would otherwise leave the outflow obstructed. It is a longer and more involved operation than a standard valve replacement.
