Removes most or all of the pericardium, the fibrous sac around the heart, when it has thickened and stiffened and is stopping the heart from filling, with the patient supported on a heart-lung machine throughout. Having the machine take over the circulation lets the surgeon peel away tissue that is stuck tightly to the heart. Releasing the constriction lets the heart fill normally again.
