An opening is cut in the pericardium, the tough sac surrounding the heart, so that fluid trapped inside it can drain away, and a piece of the sac is often removed so the opening does not seal over. Fluid building up in this space presses on the heart and stops it filling properly. This is an open surgical version rather than drainage through a needle.
