Through small cuts between the ribs and with a camera to see inside the chest, an opening is made in the pericardium, the sac surrounding the heart, so that trapped fluid can drain away, and part of the sac may be removed so the opening stays open. Fluid building up in this space presses on the heart and stops it filling properly. The chest is not opened for this.
