The worn disc between them is cleared out and a spacer packed with bone graft is set into the space, while further graft is laid alongside the back of the spine, so bone grows across and the two vertebrae become a single solid unit. It is used for a slipped or unstable segment, or where a collapsed disc is pinching nerves and other treatment has not helped.
