A self-contained pacemaker roughly the size of a large capsule is placed directly inside the lower chamber of the heart, delivered on a catheter passed up a vein from the groin. Because the whole device sits within the heart, there is no wire running to a battery pack and no pocket cut in the chest. Replacing a leadless pacemaker already in place is covered the same way.
